Important information from the Nurse's Office:
- All 6th graders and students new to Lexington Public
Schools need a CURRENT physical on file in the Nurse's
Office.
- Any student who wishes to play sports or participate in
intramurals MUST have a current physical on file. NO EXCEPTIONS!!
This is mandated by the MIAA (Massachusetts Interscholastic
Athletic Association).
- All 7th graders need a current Tetanus Diphtheria Booster
on file.
- If your child needs to take any medication in school,
please click on the link below to download the necessary forms.
Forms are needed for both prescription and over-the-counter
medications.

JUST A FYI From THE NURSE
- When students go on field trips, the only
medications I am able to send are the EPIPENS, (not the Benadryl),
INHALERS and any Prescribed DAILY MEDS.he Massachusetts Dept. of
Public Health sets regulations and we are unable to send the
Tylenol and motrins that children have for routine headaches,
etc.
